If you do not have a scanner or would rather have PEHR do the scanning for you, then mail a hard copy (never send originals, keep those for yourself) of the report (e.g. EKG, lab report, hospital report, etc) to:

PEHR will then scan the documents and
- Load them onto your "Backup Patient Data" file that you emailed to PEHR (see Custom Transcription Instructions above), or
- If you would just like the scanned document file, PEHR will email that to you and you can load it yourself onto your Bartlett.
Do not send actual X-Rays. Ask your hospital for digital copies. Check the FAQs on the PEHR website, if you need help in loading them on your Bartlett.
All printed material sent to PEHR will be destroyed after 90 days, it will not be stored or returned to you (so keep original for yourself before mailing copies to PEHR).
